8 Count Audio Liability Waiver and Release Form For Participating In The All-State Dance Routine

Please read the following statements carefully and agree by signing electronically. These agreements for participation in our programs affect your legal rights.
Certification of Authority: I, the undersigned, hereby certify that I am the Parent or Legal Guardian of the Participant(s) for whom I am completing this Liability Waiver & Release Form.
Informed Consent: I hereby give my consent for the named Participant(s) in this registration to participate in any and all activities related to the preparation and performance of the All-State Dance Routine at the All-State Dance Team Celebration, an event produced by 8 Count Audio. I understand that the very nature of dance carries a risk of physical injury. No matter how careful the instructors, staff, and participants are, the risk of injury cannot be eliminated. Injuries may occur, including minor injuries and, although rare, catastrophic injuries or death. I fully understand these risks and agree to not to hold 8 Count Audio or its instructors and staff, or staff of the hosting premises, responsible for any form of injury that may occur at any time during any activity that 8 Count Audio participates in or holds. I hereby waive, release, absolve, and indemnify 8 Count Audio, instructors, staff, and the premises staff on which any of its activities may be held, for any claim.
Identity Release and Waiver: In consideration of being allowed to participate in any way in any of the 8 Count Audio athletic programs and related activities, the undersigned:
A. Acknowledges and fully understands that each participant will engage in activities involving risk of serious injury, including permanent disability, death and severe social and economic losses which might result from their own actions, inaction or negligence of others, or condition of the premises or any equipment used. Further, that there may be other risks not known to 8 Count Audio or reasonably foreseeable at this time.
B. Assumes all the foregoing risks and accepts personal responsibility for damages which may hereinafter occur following such injury, permanent disability or death.
C. Releases, waives, discharges and covenants not to sue 8 Count Audio, its instructors and staff, other participants, and staff of the premises used to conduct an event, all of which are hereafter referred to as “RELEASEES” from demands, losses or damages on account of injury from future participation, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the “RELEASEES” or otherwise.
D. Agrees to carry personal health insurance in case of a sports injury.
E. RELEASES THE “RELEASEES” AND EXPRESSLY ASSUMES THE RISK OF INJURY AND DAMAGES; and will indemnify and hold harmless the “RELEASEES” as to any claims for injury and damage.
Hold Harmless: I release and hold harmless 8 Count Audio, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant(s) and/or the undersigned, while in or upon any premises under the control and supervision of 8 Count Audio, or in route to or from any of said premises.
Medical Treatment: I hereby give my permission for any and all medical attention necessary to be administered to Participant(s) in the event of any accident, injury, sickness, etc., under the direction of any staff member of 8 Count Audio or the hosting premises until such time as I can be contacted. I also hereby assume the responsibility for payment of any such treatment.
Media Release: The undersigned grants to 8 Count Audio permission to utilize Participant(s)’s image on any reproduction for marketing purposes, including but not limited to social media, website, print, and video, if the undersigned marks “I consent” in the corresponding box at the end of this form. By marking “I do not consent,” 8 Count Audio will not use Participants image on any reproduction for the aforementioned marketing purposes.
Infectious Diseases: By the signing of this waiver and release from liability, the undersigned acknowledges the contagious nature of infectious diseases, and voluntarily assumes the risk that they may be exposed to or infected by  infectious diseases by use of the property, facilities, services and/or programs of 8 Count Audio and that such exposure or infection result in personal injury, illness, permanent disability, and death.
The undersigned understands that the risk of becoming exposed to or infected by infectious diseases at, or on the premises of 8 Count Audio and the hosts of its events, may result from the actions, omissions, or negligence of themselves and others, including, but not limited to, 8 Count Audio and its staff, the hosting premises and its staff, program participants and their families, and other guests of the event. The undersigned voluntarily agrees to assume all of the foregoing risks and accepts sole responsibility for any injury to themselves (including, but not limited to, personal injury, disability, and death) illness, damage, loss, claim, liability, or expense, of any kind, that they may experience or incur in connection with the services provided by 8 Count Audio or participation in 8 Count Audio’s programs (“Claims”). The undersigned hereby releases, covenants not to sue, discharges, and holds harmless 8 Count Audio, its officers, directors, employees, agents and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. The undersigned agrees that this release includes any Claims based on the actions, omissions, or negligence of 8 Count Audio and its staff, whether an infection due to infectious disease occurs before, during, or after participation in the services provided by 8 Count Audio or use of the property, facilities, services, and/or programs of 8 Count Audio.
Electronic Signature: I consent to have my electronic signature on this document treated as a full and complete, legally binding signature, representing my agreement to the statements contained in this document. I realize that my electronic signature herein affects my legal rights.
